Ceftriaxone is a broad-spectrum semi-synthetic third-generation cephalosporin with a potent bactericidal activity against a wide range of gram-positive and gram-negative bacteria. Sulbactam is ß lactamase inhibitor.

The bactericidal activity of ceftriaxone results from inhibition of bacterial cell wall synthesis. Ceftriaxone exerts in vitro activity against a wide range of Gram-negative and Gram-positive micro-organisms. Ceftriaxone is highly stable to most beta-lactamases, both the penicillinases and cephalosporinases, of Gram-positive and Gram-negative bacteria.

Sulbactam irreversibly blocks the destruction of beta-lactam ring of Ceftriaxone by these wide variety of ESBLs and chromosomally mediated beta-lactamases by attaching to these enzymes and acting as a suicide substrate that forms a stable intermediate, rendering the enzyme inactive.

Okacef-SB is contraindicated in patients with known allergy to penicillins and cephalosporins. Warnings Serious or occasionally fatal anaphylactic reactions have been reported in patients receiving beta-lactam antibiotics. These reactions are more likely to occur in individuals with a history of hypersensitivity reactions to multiple allergens.
